Job Description

The Licensed Clinical Social Worker (LCSW) provides support to the Ambulatory Specialty Clinics including but not limited to PM&R, Orthopedics, and Spine Clinics.

The LCSW assists patients of all ages, and families/caregivers in coping with the social/emotional issues and practical arrangements related to the patient's illness, as well as promoting the delivery of effective, cost-efficient, quality patient care. The LCSW delivers age-appropriate social work care to patients and their caregivers per agency policy and procedure and state and federal regulations, serving as an integral member of the healthcare team providing assessments, coordination, treatment planning, information, and referral to community resources and other social work services to meet the complex needs of patients and families in the hospital and clinic settings.

Minimum Qualifications

  • Possession of an active and unrestricted CA Clinical Social Worker License (LCSW)
  • Master's Degree from an accredited School of Social Work.
  • Clinical work experience in a social service or healthcare setting (i.e., case management or social work)
  • A demonstrated ability to work with a multidisciplinary team
  • Skills, knowledge, and ability to perform psychosocial assessments on pediatric/adult/geriatric populations
  • Knowledge of suicidal behavior, ability to assess lethality, and the ability to develop an appropriate treatment plan
  • Knowledge and understanding of individual/child/family development, human behavior and communication, the influence of culture and values in social work practice, case management, and the relationship between social work practice and medicine
  • Ability to work independently with minimal supervision, make independent judgments and have decisions be reviewed if they represent a significant departure from departmental goals
  • Ability to organize a variety of concurrent activities, set realistic goals/objectives, plan, and meet deadlines without compromising quality
  • Knowledge of community resources
  • Knowledge of chronic and acute disease and how it influences individual and family functioning
  • A demonstrated awareness of, and sensitivity to, dealings with cultural and socioeconomic diversity
  • Basic understanding of computer and EMR functionality
  • Knowledge of child, elder abuse, and domestic violence reporting requirements, and other significant legislation and laws regulating social work practice, e.g., Tarasoff, patient confidentiality, etc.
  • Ability to make appropriate interventions based on law and policy
  • Good verbal and written communication skills; be able to hear and understand sufficiently via the telephone; and ability to see sufficiently to obtain information from paper and computer sources

Preferred Qualifications

  • A valid driver's license as the position requires the use of a personal vehicle in work-related activities and in-person meetings approximately 1x per quarter
  • Behavioral Health experience
  • Evidence-based therapies experience

Physical Demands and Work Environment

  • This position requires the use of a personal vehicle in work-related activities (i.e., for travel to in-person meetings, traveling between clinic locations, etc.)
  • This position performs duties at assigned work locations and shifts, with occasional overtime and/or flexed schedule as needed to accommodate emergencies
  • Working locations include but are not limited to the Ambulatory Care Center
  • Work is performed with many interruptions during the day
  • Must be able to work effectively within a stressful environment with rapidly changing priorities and deadlines
  • Must be able to maintain department attendance standards

Special Requirements

  • This position is a critical position and subject to a background check and drug screen. Employment is contingent upon the successful completion of background investigation including criminal history, identity checks, and drug screen.
