Job Summary

The Clinical Social Worker III (CSW III) is a licensed clinician providing psychosocial assessment, crisis intervention and 5150 services, individual and group therapy and substance abuse services, brief therapeutic interventions, consultation, education, community linkages, supportive services, and facilitates the plan of care for patients with particular focus on high-risk populations. This position involves much outreach to the community including client homes, hospitals, shelters, and jails as well as to the streets. The CSW III acts as a consultant to the clinical team, service lines, community partners, participates in program development and quality improvement initiatives. The incumbent will demonstrate cultural sensitivity in working with patients regardless of race, gender identity, religious affiliation, cultural beliefs, sexual orientation, etc. The incumbent will practice within the scope of UCSF's mission statement: Caring, Healing, Teaching, Discovering; and the values statement of UCSF's Everyday PRIDE: Professionalism, Respect, Integrity, Diversity, Excellence.
Under the general supervision of the Clinical Supervisor, the CSW III works within the new Citywide Community Assistance, Recovery, & Empowerment (CARE) Court Team under the Citywide Stabilization Program. We provide comprehensive intensive case management (ICM) to adults who are severely mentally ill and have a diagnosis in the schizophrenia spectrum and other psychotic disorder class who have entered into a Care Agreement to participate CARE Court. The primary objective of our program is to offer intensive outpatient support to individuals with the aim of enhancing their quality of life, reducing the likelihood of further psychiatric hospitalizations and incarcerations, and facilitating their access to appropriate ongoing treatment services. Through personalized CARE plans and dedicated support, the program endeavors to empower individuals to manage their mental health challenges effectively and to facilitate their integration into suitable pathways for sustained treatment and support. The CSW III will work collaboratively with the Adult/Older adult System of Care within Behavioral Health Services to develop creative treatment strategies. As employees of UCSF and contractors with the City of San Francisco we are essential workers and will continue to support and provide direct service during a natural disaster and or pandemic, this includes deployment which can include a temporary change in duties.
The CSW III has advanced clinical knowledge and skills and is distinguished from the CSW II by leadership activities including participation in advanced program planning, provision of educational programs, clinical supervision, consultation to the clinical team, other departments, and the community. This CSW III will support the clinical supervisors and Program Director with the management of referrals, intakes and assigning of new clients. The CSW III will support the development of new workflows and advancing the decision tree of options for vulnerable patients. CSW III will take lead along with Clinical Supervisor to train and onboard new staff. Additional CSW III duties include but not limited to monitoring data, participation in DPH State Performance Improvement Plans, keep stock of client resources, spearhead special projects and lead community case conferences.

Department Description

Citywide Case Management Programs is a Division of the UCSF/ZSFG Department of Psychiatry, located at 982 Mission St. Its mission is to provide behavioral health intensive case management services to adults who suffer from serious mental illness, co-occurring substance abuse disorder and chronic medical conditions, experience significant systemic determinant of health, including justice-involvement or other forms of institutionalization, poverty and housing instability.

Required Qualifications

  • Master's degree from an accredited school of social work or related field, possession of a clinical social worker license/ Marriage and Family Therapist license issued by the California Board of Behavioral Science Examiners or, and three years of post-master's experience; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
  • Able to work as a team member in a culturally diverse setting.
  • Ability to work with clients and program staff from diverse cultural and social class backgrounds.
  • Experience (internship/career) in a healthcare and/or hospital setting preferred
  • Knowledge and abilities essential to the successful performance of the duties assigned to the position.
  • Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.
  • Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high-volume work environment.
  • Ability to work effectively and collaboratively with interdisciplinary teams.
  • This position requires flexibility to orient and work at all UCSF locations
  • Demonstrated excellent attendance and reliability.
  • An understanding of mental health issues and sensitivity toward mental health clients.
  • One year experience doing community-based case management and the ability to do outreach
  • Two years working with severely mentally ill adults, the homeless and dual diagnosed populations
  • A preliminary job offer and assignment may be extended pending successful background clearance.
  • Able to lift 50lbs and climb stairs.

Preferred Qualifications

  • Prior related UC experience

License/Certification

  • Current California State License of Clinical Social Work or Marriage and Family Therapists and in good standing. Ability to work effectively and collaboratively with interdisciplinary teams.
